Endovascular Reconstruction of Severely Calcified Total Distal Bifurcation

Case and Plan

76-year-old male with past medical history of CAD s/p PCI, hypertension, hyperlipidemia, aortic stenosis, and ESRD on HD presenting with complaints of pain in the bilateral lower extremity thigh and calf (typical claudication) on walking 1 block. States that this has been ongoing for over 6 months and progressively worsening. Currently with resting leg complaints for the past 4-6 weeks noted as per patient persistent on conservative therapy.

ABI: Bilateral severe Obstructive PAD with Right ABI: 0.15 and Left ABI: 0.17

Arterial Doppler: Shows Evidence of Bilateral Occlusive Iliac Disease
