Case and Plan:
85-year-old male with history of hypertension, hyperlipidemia, statin intolerance, autoimmune hemolytic anemia, atrial fibrillation, CAD s/p PCI for LM-LAD and BAV on 6/14/2025 and mild frailty, now presents with progressive shortness of breath on exertion. Recent echo revealed LVEF of 60%, severe valvular aortic stenosis (PG/MG/PV/AVA: 64/42/4/0.6), mild AR and mild MR. CTA showed severe AS with calcium score 2762, mean aortic annulus of 24.4 mm, annular perimeter 76.8 mm and annular area 452.3 mm2. His STS PROM score for SAVR was 4.69% and Heart Team evaluation found him to be high surgical risk. Now planned for transfemoral TAVR via right percutaneous femoral arterial access using a 27 mm NAVITOR Vision valve.
