Case and Plan:

75-year-old male with a history of hyperlipidemia, monitored aortic stenosis, macular degeneration, obesity (BMI 30.6 kg/m²) current cigar smoker (2-4 / week), and asthma on inhalers. Recent echo showed LVEF 70%, progression to severe valvular aortic stenosis (PG/MG/PV/AVA:57/32/3.7/0.8) and mild MR. CTA showed severe AS with calcium score 1635, mean aortic annulus of 25.2 mm, annular perimeter 79.7 mm and annular area 493 mm². His STS PROM score for SAVR is 1%. Heart Team evaluation found him to be appropriate for TAVR. Now planned for transfemoral TAVR via right percutaneous femoral arterial access using a 29 mm Evolut FX+ valve.
