Patient is a 74 year old gentleman with lifestyle-limiting ischemic rest pain (L>R as well as non-healing ulcer on left foot (Rutherford Grade 3, Category 6) (WIFi Stage
4) despite GDMT and supervised exercise therapy. Work-up included venous mapping without adequate veins
Past Medical History: Primary HTN, pAF, Hypercholesterolemia, Type II DM, ESRD on iHD, History of right AKA, HFmrEF
Social History: Former Nicotine Use Disorder
Medications: ASA 81 mg, Eliquis 2.5 mg, Atorvastatin 40 mg, Metoprolol Succinate,
Losartan
