Case & Plan: 80-year-old female with HTN, HLD, Left Hip Replacement (2022), with progressive exertional dyspnea, NYHA III symptoms and intermittent dizziness for the past 1 year. Echocardiogram 4/30/25 showed Severe AS (AVA 0.9 sqcm, PG 56 mmHg, MG 34, PV 3.16 m/s) with LVEF of 60%. On CT analysis the aortic annulus measured 21.1mm x 24.4 mm (mean 22.8 mm), annular perimeter 72.1 mm and the annular area 405.2 mm2. The STS mortality risk for surgical AVR was 2.31%. The patient underwent Heart Team evaluation and was selected for TAVR. Patient now presents for TAVR with 26 mm Evolut FX+ via right femoral percutaneous approach.
