82 year-old male presented with right foot 4th toe non healing ulcer (dorsal aspect) (Rutherford Grade III – Category V, Fontaine Stage IV, Wifi 1,2,1). His medical history included recent NSTEMI, underwent PCI of LAD 1 week ago, hypertension, insulin dependent Diabetes Mellitus, atrial fibrillation and dyslipidemia. His current medications include Aspirin, Plavix, Lopressor, Insulin, Crestor and antibiotics. He underwent resting Ankle Brachial Index (ABI) which was 0.3 on right lower extremity and 0.7 on left lower extremity. During PCI of LAD, right lower extremity angiogram was performed that showed long segment occlusions of RAT and RPT. Now planned for Percutaneous Transluminal Angioplasty of chronic total occlusion of RPT and RAT.
