Multilevel Complex Intervention of Common Femoral and Superficial Femoral Artery – September 2024

Case and Plan:
HPI: 78-year-old female referred from OSH for bilateral lifestyle-limiting claudication progressive to ischemic rest pain (R>L) with < 1 block of activity. (Rutherford Grade II, Category 4) despite GDMT and supervised exercise therapy
PMHx: PAD, IDDM, HTN, HLD, HFrEF EF 25%, hx of MI s/p PCI to LAD
PAD hx: R SFA intervention with DES w/ supera stent
Social Hx: current smoker
Medications: Xarelto 2.5mg bid, Plavix, Empagliflozin, insulin, entresto, Metoprolol, atorvastatin (no cilostazol 2/2 CHF)
