56-year old male with past medical history with diabetes, coronary disease, hypertension. Status post ICS. Now on non-healing ulcer with right dorsum of the foot. Underwent amputation of the 4th toe with subsequent non-healing ulcer. Angiogram revealed multi-level disease of SFA and tibials. Plan for multi-vessel revascularization.
