Case and Plan:
82-year-old female with CCS Class III angina and positive stress MPI test showing moderate to severe apical defect. A Cardiac Cath on June 30, 2025 revealed 3 V calcified CAD including mid LAD subtotal occlusion with 90% LAD-D2 bifurcation with SYNTAX Score of 26. Patient underwent successful PCI of LCx-OM1 using atherotomy and DES. Patient is now planned for IVUS guided PCI of LAD-D2 bifurcation using RotaCut approach and Mini-Crush stenting.
