Case and plan :
A 74-year-old female presented with CCS III angina, exertional angina
with positive stress MPI for multivessel ischemia. A Cardiac Cath @ OSH
revealed calcified 3 V CAD: 80% calcified proximal LAD with 95% D1
bifurcation (Medina 1,1,1), 90% proximal LCx, 80% proximal RCA, 90%
RPDA with normal LV function and SYNTAX score of 28. After Heart
Team discussion, CABG was recommended but declined by the patient.
The patient is now planned for complex IVUS guided PCI of LAD/D1
bifurcation using rotational atherectomy and mini-crush technique
