Case and Plan:

Demographics: 90-year-old male

Presentation: CCC Class III angina

CAD Risk Factor: Hypertension, Hyperlipidemia, T2DM, Mild thrombocytopenia, hypothyroidism, BPH, cirrhosis, chronic rectal bleeding

Coronary Angiography: 90-95% ulcerated severely calcified mid LAD, and 80-90% severely calcified proximal RCA lesions. EF 55%. SYNTAX Score 23.

Plan: IVUS-guided PCI for the severely calcified LAD lesion
