ID: We have an 86-year-old gentleman who presented for an enlarging left common iliac artery aneurysm

PMHx: Bilateral common iliac artery aneurysms s /p right common iliac artery stent graft 4-years ago Right internal iliac artery aneurysm s/p coiling 4-years ago Severe aortic stenosis s/p TAVR Coronary artery disease s/p multiple PCI Stage Ill Lung cancer s/p XRT and immunotherapy in remission

Social Hx: 40-pack year smoking history, quit 10 years ago

Medications: Aspirin 81mg daily, Atorvastatin 40mg daily, Clopidogrel 75mg daily, Ezetimibe 10mg daily, Furosemide 20mg daily, Lisinopril 5mg daily, Metoprolol succinate 25mg daily, Pantoprazole 40mg daily

History of Present Illness: The patient was evaluated 4-years ago for bilateral claudication and found to have bilateral iliac artery aneurysms (>3.5cm on the right with internal iliac artery aneurysm, along with 2.5cm left common iliac artery aneurysm). He underwent coiling of the right internal iliac artery and stent graft placement into the right common iliac artery.

The patient was lost to follow up for 4 years. He was referred to our clinic after repeat imaging found the left common iliac artery aneurysm had progressed to 3.5cm in size, along with an abdominal aortic aneurysm of 3.4cm.
