85-year-old gentleman with lifestyle limiting claudication (L greater than R) that has progressed to ischemic rest pain despite optimal medical therapy (Rutherford II, 4)
PMH: Primary HTN, HLD, CAD s/p multiple PCIs, Chronic Systolic HF/Ischemic, Cardiomyopathy, Type II DM, PAD s/p multiple interventions, COPD, Chronic Venous, Insufficiency s/p ablations
Social: Former Nicotine Use Disorder (105 pk yr smoker), no other substances utilized
Medications: Aspirin, Clopidogrel, Rosuvastatin, Ezetimibe, Lisinopril, Metoprolol Succinate, Spironolactone, Furosemide, Empagliflozin, Metformin, Inhalers
