Case and Plan:

75-year-old female who presents for intervention of aortoiliac disease and renal artery stenosis. Patient has polyvascular disease and was seen in clinic with poorly controlled primary hypertension and lifestyle limiting claudication that has progressed to ischemic rest pain (Rutherford Grade II, Category 4) from the hip and buttock area radiating down to the legs and underwent peripheral angiogram and renal angiogram notable for coral reef aorta with aortoiliac disease and right renal artery stenosis.

PMHx: Primary HTN, CAD s/p PCI with DES to mLAD and pRCA, Carotid Artery Stenosis, Chronic Systolic HF, Moderate Aortic Insufficiency, Lung Cancer s/p left lung resection

Social Hx: Former Smoker

Medications: Aspirin, Plavix, Valsartan, Carvedilol, Amlodipine, Chlorthalidone, Rosuvastatin, Ezetimibe
