Case and Plan:
74 year-old male referred for persistent lifestyle limiting claudication progressive to ischemic rest pain (L greater then R), Rutherford Grade III Category 5. Has had multiple toe amputations of bilateral feet prior.
PMHx: HTN, HLD, IDDMII, ICM, CAD s/p multiple PCIs, PAD s/p multiple interventions
Social Hx: Former smoker
Medications: Aspirin, Ticagrelor, Atorvastatin, Carvedilol, Entresto, Dapagliflozin, Linagliptin, Metformin, Insulin
