Case and Plan:
83-year-old male with severe COPD presented with CCS Class IV angina and normal troponins. A Cardiac Cath on June 6, 2025 revealed calcified 3 V CAD: 100% proximal RCA, multiple 80-95% LAD lesions with subtotal large D1, 95% proximal LCx-OM1 with SYNTAX Score of 43 and EF 65%. After Heart Team discussion, CABG was declined due to COPD and lymphoma and underwent successful PCI of LCx-OM1. Patient is now planned for staged PCI of multiple long calcified LAD bifurcation lesions using RA and Mini-Crush technique.
