ID: An 80-year-old gentleman with multiple comorbidities presented to our clinic for evaluation or severe, bilateral lifestyle limiting claudication, resistant to medical treatment and exercise therapy
PMHx: HTN, HLD, CAD s/p multiple PCI, HFrEF with ischemic cardiomyopathy, LVEF 20-25%, severe aortic stenosis s/p TAVR, Severe COPD on home oxygen, prior tobacco use, renal artery stenosis s/p right renal artery stenting, CKD stage 3b, and PAD with prior R SFA and L SFA stenting
Social Hx: Former smoker (50 years, quit 5 years ago)
Medications: Aspirin, Amlodipine, Entresto, Empagliflozin, Furosemide, Metoprolol Succinate, Rivaroxaban
2.5mg BID, Rosuvastatin, Spironolactone
