Case & Plan:
68-year old male with known CAD and prior PCI’s presents with CCS Class III angina with positive SPECT MPI. A Cardiac Cath on June 24, 2021 revealed calcified 2 V + LM CAD: 70% calcified dLM, 80% severely calcified tortuous proximal LAD, 100% LAD-D1, 60% LCx-OM1, 95% Ramus Intermedius and SYNTAX Score of 40 and LVEF = 60%. Patient was referred for CABG, but declined by the family. Patient is now planned for high-risk multivessel PCI using rotational atherectomy and IVUS imaging for optimization.
