Present Clinical Presentation

Presented with new onset CCS class I| angina/ dyspnea and MV ischemia on PET-CT scan

Hypertension – controlled

Hyperlipidemia – controlled

Clinical Variables

SAQ-7 score: 77, + F/H

Known CCS for 2 decades, s/p PCls x2 10 yrs ago,

HbA1c:6.1/ LDL:48/ CRP:0.5/ SCr:0.8, eGFR:96 echo LVEF 50%

Medications

Aspirin, ISMN, Metoprolol XL, Entresta (Vyamada), Atorvastatin, Ezetimibe, Metformin, Spironolactone Cath: Cath on October 7h 2025 revealed calcific 2 V CAD of LAD-D1 bifurcation & LCX-OM2/LPDA with syntax score 26 and LVEF 60%. Pt underwent successful RotaDES PCI of LCX-OM2 & did well. Clopidogrel 75mg daily added.

Plan Now planned for HD-IVUS guided staged complex PCI of calcified

LAD/D1 bifurcation using RotaCut plaque modification and Mini-crush stenting.
