Present Clinical Presentation:
Presented with new onset DOE and CCTA with high Ca++ score of 1677 with +ctFFR in all 3 vessels
CAD Risk Factors:
Hyperlipidemia – controlled
SAQ-7 score: 65
HbA1:5.5/ LDL:56/ CRP:1.6/ SCr: 1.2, eGFR:64
Clinical Variables
No known CAD, echo LVEF 60%
Medications
Aspirin, Rosuvastatin
Cath:
Cath on Feb 27th, 2026 revealed calcific 3 V+LM CAD with multiple lesions of LAD and RCA with syntax score 35 and LVEF 60%. After Heart team discussion, pt underwent successful Atherotomy+DES PCI of RCA and did well. Clopidogrel 75mg daily and Metoprolol XL 25mg added.
Plan:
Now planned for HD-IVUS guided staged complex PCI of distal LM bifurcation using Mini-crush technique and RotaDES to other lesions.
