Patient Demographics:
92 yrs, Female

Present Clinical Presentation:
Presented with CCS Class III angina and DOE

CAD Risk Factors:
Controlled Hypertension, Hyperlipidemia
LDL: 76, SCr: 0.79, HbA1c: 5.3

Clinical Variables:
Severe AS post BE-TAVR, EF 42% on echo.

Medications
Aspirin, Atorvastatin, Empagliflozin, Amlodipine, ISMN, Furosemide

Cath:
Cardiac cath on 10/20/2025 @MSH revealed severe calcified 90-95% proximal to mid RCA lesions; 70-80% mid-LAD lesion, 90% Diagonal lesion, and total occlusion of LCX. Syntax score 34. Patient has successful transfemoral TAVR with 26mm SAPIEN-3
Ultra Resilia. Pt is active and continues to have exertional dyspnea. ISMN added.
