Case and Plan:
Demographics: 69-year-old male
Presentation: CCS Class III angina with positive SPECT-MPI
CAD Risk Factor: Hypertension, Hyperlipidemia, and NIDDM
Coronary Angiography: Left main and three-vessel CAD, including a 60-70% LM lesion, 80% proximal and 90% mid-LAD ISR, an 80% proximal LCx lesion, and a 50% RCA lesion. LVEF was 60%, and the SYNTAX Score was 32. The patient was referred for CABG evaluation but declined surgery. Pt subsequently underwent successful LCx PCI with DES implantation.
Plan: IVUS-guided LM assessment and PCI of diffuse LAD ISR
